So, if I legally live in Washington State, work for a company in Maryland, am staying some number of months in Pennsylvania, and am buying a car from a cow-orker in Virginia... which state does it need inspection and licensing for?

Pennsylvania. You'll have to go through the whole rigamaroll again in Washington when you come back, providing you wish to keep it after said time.
